        As to DEBORAH COOPER BURG, for the months of June, July and August 2019,
        for all times DEBORAH COOPER BURG was a patient at FAWCETT MEMORIAL
        HOSPITAL, please produce the actual pages from DEBORAH COOPER BURG’S
        medical records from FAWCETT        MEMORIAL HOSPITAL in your response to this
        request that relate to the following:
