affirmation of Robert R. Dooley, Esq., the affidavit of Daryl Squicciarini, and Exhibits attached
thereto, the Plaintiff, Daryl Squicciarini, will move this Court at the Courthouse located at 1 Court
Street, Riverhead, New York, on December 21, 2023 at 9:30 a.m. in the forenoon or as soon
thereafter as counsel may reasonably be heard, for an order pursuant to C.P.L.R. § 3213 granting
Plaintiff summary judgment in lieu of a Verified Complaint in the amount of $[insert] against the
Defendant upon the ground that this action is based upon an instrument for the payment of money
only which is now due and payable, for an award of attorneys’ fees, and for such other and further
