testimony in this action and that you bring with you, and produce at the time and
place aforesaid, (IN LIEU OF PERSONAL APPEARANCE, forward records to Supreme Court,

10007. Attach a copy of the subpoena with records.)
CERTIFIED and complete copies of each and every page of each and every
department of the hospital including but not limited to E/R records, treatment
records, x-ray reports, RAY PLATES, ans, Surgical Reports and other
test reports, photographs, physical therapy records, clinics records, diagnosis
sheets, pathological reports, consultation reports, laboratory work-ups, etc
as well as a CERTIFIE! HOSPITAL BILL without reference to the source of
payments for the following patient

Failure to comply with this subpoena is punishable as a contempt of Court and
shall make you liable to the person on whose behalf this subpoena was issued for a
penalty not to exceed fifty dollars and all damages sustained by reason of your
failure to comply
