Defendant, SAMUEL ESTEPA, M.D., requests the Plaintiff to produce for inspection
or copying the following:
1. Copies of Income Tax Returns for decedent for the year of his death and the
preceding six years, and for and each "survivor" for the years 1995, 1996, 1997, 1998, 1999, and
evidence of income in 2000.
2. Photographs showing decedent's condition during the one (1) year period
preceding his death and any photographs taken while decedent was hospitalized or at the funeral.

3. Copies of all medical bills, hospital and/or nursing bills, drug bills, dental
bills, and all other bills supporting Plaintiff's claim for damages.
4. Copies of all existing medical, dental, and hospital records and reports of
decedent and each survivor for the ten (10) years prior to his death.
5. All business records, pay receipts and other memoranda in the possession.
custody or control of the Plaintiff, indicating the gross income for decedent and each survivor from
self-employment, salary, commissions, bonuses, overages, credits reimbursements, pensions or social
security for the five (5) years before Mr. Simons’ death.
6. "Statements" of the Defendant, Samuel Estepa, M.D., or his agents and
employees.
7. Reports of any and all "expert" witnesses employed or retained by you, your
agents, servants, or attorneys, which you intend to use as witnesses.
8. All documents, reports, correspondence or other written memoranda
expressing opinions of any health care provider pertaining to the care and treatment rendered to
decedent.
9. All documents concerning any other legal actions in which the decedent or
any survivor has ever been involved in either as Defendant or Plaintiff.
10. Copies ofany "Mary Carter" Agreements, loan receipt agreements, settlement
agreements, releases, or covenants not to sue, agreements to dismiss or any agreements of any kind
entered into by you, or on your behalf, with any other defendant, person, firm, or corporation who
you contend may have some liability exposure to the incident complained of. This includes letters
or memoranda outlining oral agreements.
fl. Any and all documents including insurance policies or benefits booklets
evidencing payments made or to be made to the Plaintiff or on their behalf by or pursuant to the
following:
a. United States Social Security Act;
b. Any federal, state or local income disability act;
c. Any other public program providing medical expenses, disability
payments or other similar benefits;
d. Any health, sickness or income disability insurance;

e Any automobile accident insurance that provides health benefits or

f. Any other insurance benefits, including life insurance benefits,
available to Plaintiff whether purchased by her or provided by others;
g. Any contract or agreement of any group, organization, partnership or
corporation to provide, pay for, or reimburse the cost of hospital, medical, dental or other health care
services;
h. Any contractual or voluntary wage contribution plan provided by
employers or any other system intended to provide wages during any period of disability.
12. A copy of decedent’s will.
13. A copy of the inventory filed for decedent's estate.
14. Any autopsy report.
