1 The Plaintiffs requested specific policies, procedures, protocols, rules and
regulations from the defendant nursing home rehabilitation facility, LIFE CARE CENTER OF
PUNTA GORDA, as follows (See attached Composite Exhibit A):
1 For the month of August 2019, please produce all
policies and procedures, protocols, rules and
regulations concerning the following for your
patients/residents:
A Food and nutrition services
B. Laboratory, radiology and other diagnostic
services
Pharmacy services
Nutritionists and/or dietitians on staff as well
as their role and responsibilities
Therapeutic diets
Menus and nutritional adequacy
Vitamin deficiency recognition and testing as
well as administration of vitamins
Providing of specialists to see
patients/residents such as neurologists and
ophthalmologists
I Identification of patients/residents’ need for
vitamin and/or artificial nutrition
J Nutritional assessment of patients/residents
2 The Defendants objected to same and then made a partial production of a
Table of Contents so that Plaintiffs cannot tell if materials are being held back (See attached
Composite Exhibit B). Further, chapters in manuals are incomplete and leave out portions
of manuals such as how to utilize the manual, definitions, glossaries, indexes and the like.
3 The undersigned has attempted to confer with counsel for said Defendants and
will continue these attempts in an effort to avoid the hearing.

Services Manual, pursuant to the Confidentiality Agreement signed by the Plaintiff.

Laboratory, radiology and other diagnostic services Pharmacy services Nutritionists and/or dietitians on staff as well as their role and responsibilities Therapeutic diets Menus and nutritional adequacy Vitamin deficiency recognition and testing as well as administration of vitamins Providing of specialists to see patients/residents such as neurologists and ophthalmologists I Identification of patients/residents’ need for vitamin and/or artificial nutrition J Nutritional assessment of patients/residents 2 The Defendants objected to same and then made a partial production of a Table of Contents so that Plaintiffs cannot tell if materials are being held back (See attached Composite Exhibit B). Further, chapters in manuals are incomplete and leave out portions of manuals such as how to utilize the manual, definitions, glossaries, indexes and the like. 3 The undersigned has attempted to confer with counsel for said Defendants and will continue these attempts in an effort to avoid the hearing.
