                    Defendant    WEST        FLORIDA       PHYSICIAN     NETWORK,     LLC,   by   and   through

        undersigned counsel and pursuant to Florida Rule of Civil Procedure 1.350, hereby responds to
        VANCE         MALONEY,           III’S Request   for Production served November 4, 2020 and states as
         follows:
              1     A copy of the insurance policy which would cover the subject incident.

                    RESPONSE:     The requested information has been produced by Defendant, Fawcett
                    Memorial Hospital in its Responses to Plaintiff's First Request for Production and
                    its Amended Responses to Plaintiffs’ First Request for Production.
2.   Your medical records and billing concerning the Plaintiff.

         RESPONSE:      Objection: Overbroad, and not reasonably limited in time and scope.
         Subject to and as limited by the foregoing, see attached medical records from Gulf
